To request a reservation for your Dallas Baptist Association group please complete and submit the form below.

If your group or organization is not a member of the Dallas Baptist Association you must fill out an “Application for Retreat Account Form” , and sign it and fax it to 972-291-4958, in addition to the form below. Note: we must have the “Application for Retreat Account Form” on file before we may proceed with any Non-DBA reservations.

Mt. Lebanon reserves the right to refuse, accept, cancel, or alter any booking request or reservation
